Remote Therapeutic Monitoring

Client Success: Vineyard Complementary Medicine

Vineyard Complementary Medicine is a single-location physical therapy practice serving Martha’s Vineyard. Facing seasonal population surges and frequent patient travel, the clinic needed a scalable way to extend care between visits, maintain continuity, support adherence, and generate revenue through RTM.
Challenge
VCM needed a scalable way to maintain care continuity and revenue during peak seasonal population surges without relying on additional in-person staff. Many patients frequently travel, commute, or leave the island for extended periods, making consistent in-clinic visits difficult. Without a reliable way to stay connected between visits, the clinic faced challenges maintaining engagement, adherence, and ongoing clinical oversight.
Solution
VCM implemented a digital-first care model built around Remote Therapeutic Monitoring (RTM) to extend care beyond the clinic walls and support patients between visits. Telehealth evaluations helped manage new-patient intake during high-demand seasons, while RTM enabled clinicians to remotely monitor progress, guide treatment, and stay connected regardless of patient location. By integrating RTM into everyday workflows through EverEx, VCM created a scalable care pathway that strengthened adherence, supported remote treatment, and generated predictable monthly reimbursement without expanding physical space or staffing.

How Vineyard Complementary Medicine Scaled Care Beyond an Island With Digital PT & RTM Outcomes

A partnership focused on flexibility and sustainability

While Telehealth helped manage evaluations during busy months, RTM became the long-term structure that enabled VCM to support patients throughout the entire episode of care. With EverEx, the clinic could remotely track patient activity, monitor symptoms, adjust home exercise programs, and communicate with patients between visits—keeping them engaged regardless of travel, schedule constraints, or seasonal disruptions.

“It felt like RTM finally aligned with how we wanted to practice,” Dr. Sanford shared. “The workflows matched our needs, and the platform made it simple for the team.”

Immediate operational and clinical impact

RTM quickly became one of the most impactful components of VCM’s digital care strategy.

  • 77 patients enrolled in RTM

  • $14,468 in RTM revenue (June 2024–August 2025)

  • $154.61 average reimbursement per patient per month

  • 1,553 total workouts completed

  • 58.7 average app logins per patient

  • 340+ message exchanges

  • 2.62-point pain reduction and 4.75° ROM increase

“RTM kept our patients engaged between visits—especially those who leave the island, commute long distances, or struggle with scheduling in the summer,” Dr. Sanford said.

Empowering clinicians and stabilizing growth year-round

RTM created new flexibility for VCM’s clinicians as well. Therapists who moved off-island were able to continue treating patients virtually, while others took on RTM caseloads as supplemental income. RTM reduced pressure on onsite staff, maintained continuity during unpredictable seasons, and allowed clinicians to oversee patient progress without requiring additional in-person appointment slots.

“These tools reduced the strain on our team and helped us deliver care consistently,” Dr. Sanford explained. “RTM gave us the flexibility we needed to navigate the island’s seasonal changes.”

At its core, RTM allowed VCM to overcome the geographic isolation that once limited its growth.“At the end of the day, RTM lets us care for more people in our community—even when they’re not physically here.”
