Remote Therapeutic Monitoring is a set of CPT codes established by CMS to reimburse providers for monitoring patients between visits — tracking home exercise program adherence, logging pain and progress, and communicating with patients about their care.
The key phrase is between visits. RTM pays you for clinical activity that's happening outside the clinic. Activity your therapists may already be doing informally, for free.
A typical RTM patient generates $100–$150 per month in reimbursement on top of normal visit billing. For a practice with even 30 active RTM patients, that adds up to $3,000–$4,500 in new monthly revenue.
EverEx is the platform that makes it simple. We automate the tracking, engagement, and billing so your team can focus on care, not paperwork. FDA-registered, HIPAA-compliant, and built for clinics, hospitals, and independent practices.
EverEx integrates into your existing workflow so your team can start generating RTM revenue in days, not months.
During a normal visit, introduce RTM and enroll the patient through EverEx. It takes less than 2 minutes and integrates with your EMR.
Patients follow their HEP through the EverEx app, logging exercises, pain levels, and progress. AI-powered motion analysis provides precise movement insights.

Everything providers need to know about RTM, from CPT codes and billing to getting started with EverEx.
Remote Therapeutic Monitoring (RTM) is a CMS-approved framework that allows physical therapists, occupational therapists, and speech-language pathologists to remotely monitor patients' therapeutic data between clinic visits. This includes exercise adherence, pain levels, and functional outcomes. RTM uses dedicated CPT codes (98975, 98977, 98979, 98980, 98981) to create a reimbursable revenue stream for rehab therapy practices.
A typical RTM patient generates $100 to $150 per month in reimbursement on top of normal visit billing. For a practice with even 30 active RTM patients, that adds up to $3,000 to $4,500 in new monthly revenue. Real EverEx clinics have generated up to $300 per patient per episode of care and $14,468 in recurring RTM revenue.
RTM (Remote Therapeutic Monitoring) tracks musculoskeletal and therapeutic data like exercise adherence, pain levels, and rehab outcomes. RPM (Remote Patient Monitoring) tracks physiological data like blood pressure, glucose, and oxygen saturation. RTM uses CPT codes 98975 through 98981, while RPM uses codes 99453 through 99458. Importantly, PTs, OTs, and SLPs can bill for RTM but generally cannot bill for RPM.
The 2026 RTM CPT code family includes: 98975 (initial setup, $21.71), 98985 (2–15 days engagement, $51.44), 98977 (16+ days engagement, $51.44), 98979 (10–19 min virtual support, $26.39), 98980 (first 20 min virtual support, $54.11), and 98981 (each additional 20 min, $41.42). All rates are national averages.
Physical therapists (PTs), occupational therapists (OTs), speech-language pathologists (SLPs), and physicians can bill for RTM services. The ordering provider must establish the treatment plan, and monitoring can be performed by clinical staff under general supervision.
No. Unlike RPM, which requires FDA-cleared physiological monitoring devices, RTM can be delivered through an FDA-registered software platform. EverEx is FDA-registered and HIPAA-compliant, meaning patients only need a smartphone or tablet to participate. No wearables or special hardware required.
CMS requires a minimum of 2 days of data transmission within a 30-day period for the base engagement code (98985). For the higher-tier engagement code (98977), 16 or more days of data transmission are required. Everex automatically tracks transmission days and flags when billing thresholds are met.
Yes. RTM is designed to supplement, not replace, in-clinic care. RTM codes can be billed in the same month as in-person evaluation and treatment codes. However, RTM and RPM cannot be billed for the same patient in the same 30-day period.
EverEx automates the tracking, engagement, and billing so your team can focus on care, not paperwork. The platform tracks patient activity, logs data transmission days, monitors billing thresholds, and generates claims for all six RTM CPT codes. Built-in billing tools ensure accuracy and efficiency.
Yes. EverEx is fully HIPAA-compliant and FDA-registered. All patient data is stored securely, and the platform meets all federal and state requirements for protected health information (PHI).
From solo practices to hospitals and multi-location groups, EverEx helps providers track patient progress, improve outcomes, and generate recurring RTM revenue. No extra staff or complexity required.
